• Increasing MTTA over time (analysts slower to respond)
• Decreasing investigation depth (fewer actions per triage)
• Bulk closure patterns (closing multiple alerts without individual review)
• High-severity alerts receiving same triage time as low-severity
• Off-hours alert abandonment (alerts during nights/weekends going unreviewed)

Step 2.4 -- Impact Quantification

Calculate the business cost of alert fatigue:

• Analyst hours consumed by false positives per month
• Estimated true positives missed due to alert volume (detection gap)
• Cost per false positive (analyst time x hourly rate)
• Risk exposure from unreviewed alerts during peak volume periods

Step 4.4 -- Rule Lifecycle Management

Assess detection rule governance maturity:

• Rule creation: who can create, review, and approve?
• Rule testing: is detection validated before production deployment?
• Rule review cadence: periodic review schedule and adherence
• Rule retirement: when and how are obsolete rules removed?
• Version control: detection-as-code maturity level (none/basic/mature)

• L1 to L2 escalation criteria and compliance rate
