Rejecting a filing piecemeal, one deficiency at a time across multiple visits, instead of a complete list up front. Treating "this case type is usually public" as permanent status without checking for a case-specific sealing order issued later in the case's life. Approving a fee-waiver request by eyeballing apparent hardship instead of applying the jurisdiction's stated income threshold.
