Confirm type 2 diabetes (not T1D or other insulin-dependent diabetes). Assess admission blood glucose (BG) <180 mg/dL, recent HbA1c <7.5% (9.4 mmol/L). If patient was on insulin before hospitalization, verify total daily insulin dose <0.6 units/kg/day.
Rule out if patient has type 1 diabetes, BG ≥180 mg/dL, HbA1c ≥7.5%, or (if on pre-admission insulin) total daily insulin dose ≥0.6 units/kg/day. If any exclusion applies, do not proceed with DPP4i strategy.
If patient meets all inclusion criteria (T2D, BG <180 mg/dL, HbA1c <7.5%, and appropriate insulin dose if applicable), classify as a select patient suitable for DPP4i‑based approach.
Initiate either a DPP4 inhibitor (e.g., sitagliptin 100 mg daily) plus correction insulin (sliding scale) for intermittent hyperglycemia, OR start scheduled insulin therapy (basal ± bolus) if preferring insulin monotherapy. Re‑evaluate BG; if persistently >180 mg/dL on DPP4i, switch to scheduled insulin.
Do not use in T1D or insulin‑dependent diabetes. Avoid if admission BG ≥180 mg/dL or HbA1c ≥7.5%. If BG remains >180 mg/dL despite DPP4i, discontinue DPP4i and initiate scheduled insulin. Counsel on cost and discharge planning if continuing DPP4i post‑hospitalization.
A 66‑year‑old woman with T2D, home metformin, admission BG 148 mg/dL, HbA1c 7.0%, no home insulin. Meets criteria. Prescribe sitagliptin 100 mg daily plus correction insulin lispro 2 units for BG 150‑180, 4 units for 181‑250, 6 units for >250 mg/dL before meals.
