Adding EPA ethyl ester for type 2 diabetes on statin with residual hypertriglyceridemia
STEP 1 — Gather Information
Collect: type 2 diabetes diagnosis, current statin therapy, LDL-C level (confirm at goal <70 mg/dL), triglyceride level, and presence of two additional traditional risk factors (e.g., hypertension, low HDL-C <40 mg/dL men/<50 mg/dL women, smoking, family history of premature ASCVD) or risk-enhancing factors (elevated Lp(a) ≥50 mg/dL, hsCRP >3 mg/L, CKD, microalbuminuria, retinopathy, etc.). Also assess ASCVD risk if needed for context.
STEP 2 — Rule In / Rule Out
Is the patient on a statin with LDL-C at goal (<70 mg/dL) and triglyceride level >150 mg/dL? If YES, proceed to Step 3; if NO, do not add EPA ethyl ester and consider other TG-lowering strategies.
STEP 3 — Classify or Stratify
Does the patient have at least two additional traditional risk factors or risk-enhancing factors? If YES, classify as candidate for EPA ethyl ester; if NO, do not add EPA ethyl ester and manage per standard lipid guidelines.
STEP 4 — Decide
Add eicosapentaenoic acid ethyl ester 4 g/day to reduce cardiovascular risk; re-evaluate lipids and tolerability in 4–12 weeks.
Clinical Guardrails / Mimics / Pitfalls
Do not use if triglycerides ≥500 mg/dL (consider fibrate instead); avoid in pregnancy or active liver disease; monitor for atrial fibrillation and bleeding; EPA ethyl ester does not significantly lower LDL-C, so LDL goal must already be met with statin; if EPA ethyl ester unavailable, consider fenofibrate as alternative.
Clinical Example
A 58‑year‑old man with type 2 diabetes on atorvastatin 20 mg daily has LDL‑C 65 mg/dL, triglycerides 180 mg/dL, hypertension, and HDL‑C 38 mg/dL. He meets criteria: statin at LDL goal, TG >150 mg/dL, plus two risk factors (hypertension, low HDL). Prescribe EPA ethyl ester 4 g/day.
