Lipoprotein(a) measurement for family history of premature ASCVD
STEP 1 — Gather Information
Collect patient's family history of premature ASCVD, personal history of ASCVD, and family history of high lipoprotein(a).
STEP 2 — Rule In / Rule Out
If any of the following are present: family history of premature ASCVD, personal history of ASCVD, or family history of high lipoprotein(a), proceed to measure lipoprotein(a); otherwise, do not measure lipoprotein(a) for this indication.
STEP 3 — Classify or Stratify
Measure lipoprotein(a) and classify as elevated (≥50 mg/dL or ≥125 nmol/L) or not elevated.
STEP 4 — Decide
If lipoprotein(a) is elevated, consider intensifying LDL-C-lowering therapy; if not elevated, continue current lipid-lowering regimen based on overall risk assessment.
Clinical Guardrails / Mimics / Pitfalls
Lipoprotein(a) testing does not need to be repeated if previously measured; lipoprotein(a) ≥50 mg/dL is a risk-enhancing factor for ASCVD; reducing lipoprotein(a) has not yet been proven to reduce ASCVD risk; do not use lipoprotein(a) alone to guide therapy without considering overall risk; avoid measuring in patients without the specified indications as it adds little to global risk assessment.
Concrete Clinical Example
A 45-year-old man reports his father had a myocardial infarction at age 50 (premature ASCVD); he has no personal ASCVD and no prior lipoprotein(a) measurement; lipoprotein(a) is measured at 80 mg/dL; decision made to intensify his statin therapy from moderate to high intensity.
