Collect fasting triglyceride level (mg/dL), child's age (0–9 years), and confirm fasting state (≥8 hours). Record BMI percentile to contextualize obesity risk.
Is fasting triglyceride <75 mg/dL? If yes, classify as acceptable (rule out dyslipidemia); if ≥75 mg/dL, proceed to further stratification.
If triglyceride is 75–99 mg/dL → borderline high; if ≥100 mg/dL → high.
Acceptable: routine lipid screening per obesity follow‑up schedule. Borderline high: initiate lifestyle counseling (diet, activity), repeat fasting lipid panel in 3–6 months. High: intensify lifestyle intervention, evaluate full lipid panel, consider referral for specialist management if persistent.
Do not use non‑fasting samples; triglycerides vary day‑to‑day and require confirmation. Avoid diagnosing dyslipidemia on a single value—repeat abnormal results. Do not initiate pharmacotherapy based solely on triglycerides without LDL‑C/HDL‑C assessment. Consider ethnic variations in lipid norms.
A 7‑year‑old with BMI at the 92nd percentile has a fasting triglyceride of 88 mg/dL → borderline high. Clinician provides lifestyle counseling (reduce sugary beverages, increase activity) and orders a repeat fasting lipid panel in 4 months.
