Confirm intact preoperative thyroid function (normal TSH and fT4), document surgery type and date, assess for hypothyroid symptoms, and ensure at least 6 weeks have elapsed since operation.
Is the patient ≥6 weeks postoperative with intact preoperative thyroid function? If yes, proceed to measure fT4; if no, defer testing until the appropriate window.
Interpret fT4: a level below the laboratory reference range (with normal, low, or mildly elevated TSH) indicates central hypothyroidism; a normal fT4 makes CH unlikely.
If fT4 is low, initiate levothyroxine replacement targeting mid‑to‑upper reference range fT4; if fT4 is normal, continue routine follow‑up and repeat testing only if symptoms develop.
Do not rely on TSH alone to diagnose CH; avoid testing before 6 weeks post‑op due to transient thyroid changes; assess for adrenal insufficiency before starting L‑T4 to prevent adrenal crisis; avoid over‑replacement by targeting fT4 in the mid‑to‑upper reference range.
A 48‑year‑old man undergoes endoscopic resection of a Rathke’s cleft cyst; preoperative TSH and fT4 are normal. At 7 weeks post‑op, fT4 is 0.7 ng/dl (low) with TSH 1.9 mIU/L; central hypothyroidism is diagnosed and L‑T4 1.6 µg/kg/d is started.
