Set up an ADR Kit project (register, adopt, hooks, upgrade)

Select adr-kit:setup from /skills, or ask Copilot to use the setup ADR Kit skill. Resolve <plugin-root> from this installed skill. Use only bundled, local ADR Kit tools and follow this canonical workflow:

1. Setup is one entry point with four modes (spec R19): no argument runs register (this procedure); 'adopt' routes to the init workflow, 'hooks' to install-hooks, 'upgrade' to upgrade. Reject any other mode argument.
2. Run python <plugin-root>/scripts/setup-project.py --client <client-id> --project-root <workspace>, where <client-id> is this client's id and <workspace> is the project root.
3. Preserve all user content outside managed markers and .adr-kit/ADR-guide.local.md; rerunning unchanged input must be a no-op.
