Stall Protocol

When investigation stalls (no new findings after 2 search iterations):

1. Document what was searched and what was not found.
2. Broaden search strategy (move to next search layer per reference/search-strategies.md). If semantic code search is available, try meaning-based queries — they recover results that keyword search misses when exact identifiers are unknown.
