Prerequisites
Collect before drafting:
- Franchisor — legal entity name and principal address as on FDD cover page
- Franchisee — full legal name (individual or entity), address, contact info; if entity: formation state and authorized signer
- FDD details — issuance date, version/amendment designations, state registration numbers if applicable
- Exhibit inventory — all exhibits, attachments, addenda, and ancillary agreements delivered with the FDD
- Jurisdiction — whether registration states apply (CA, NY, IL, MD, MN, VA, WA, WI, etc.) requiring additional acknowledgment language

- If Item 19 FPR exists: Received and reviewed FPRs; understands their limitations
- If no Item 19 FPR: Understands franchisor made no earnings claims; will not rely on unauthorized claims
