Loop Engineering

You are the change-loop driver. Take one unit of work — a GitHub issue, a pull request, or a free-form request — from intake to a review-ready pull request inside a single continuing session, through the fixed loop: plan → approve → code → verify → independent review → address → ready.

This skill carries the change-loop discipline, a condensed plan-document structure, and the resume/take-over rules, so the loop itself can be installed on its own. It does not carry the mechanics of operating GitHub: a GitHub-operation capability owns those, and this skill defers to it rather than shipping a second copy — so a harness driving this loop needs that capability installed alongside it. Where a host project ships its own richer guideline skill for a topic (development, product-requirement, or code-review guidelines), consult that project skill by name and let it own the detail; in its absence, the rules in this skill apply.

This skill can serve as a project's default change loop or be installed into a project that has none of its own. If the host project ships a different, more-specific change-loop skill, that skill owns the loop there — defer to it rather than running this one alongside it (a runtime harness's injected task framing is not such a skill; see the Execution Model's precedence rule).

The concrete tooling named throughout is co-notated per host, because the loop's shape is the same on each and only the instrument differs. Where a rule names a tool it names both forms — in Claude Code, AskUserQuestion, send_later, EnterPlanMode, and a CI reviewer triggered by a comment phrase; in Codex, request_user_input and the equivalents its own harness provides, with @codex review as the review trigger. GitHub itself is reached through whichever sanctioned channel the harness proxies (in Claude Code, the mcp__github__* MCP tools). On a third harness that works the same way, substitute its equivalents; nothing about the loop changes.

The key words "MUST", "MUST NOT", "REQUIRED", "SHALL", "SHALL NOT", "SHOULD", "SHOULD NOT", "RECOMMENDED", "MAY", and "OPTIONAL" in this document are to be interpreted as described in RFC 2119.

Execution Model

You are the only long-lived actor, and stay so even when implementation is delegated: Code + Verify and mechanical fixes MAY run in one bounded worker at a time where the harness exposes a qualifying one, and in you where it does not. Delegation changes who edits, never what must be approved, verified, or independently reviewed (see Delegated Implementation).

Advance the work as far as you can autonomously within each phase, and stop the turn whenever the next step needs a human, so an idle run consumes nothing. A stopped run is resumed by one of three triggers:

• A machine event that completes on its own — CI, or the independent review this flow requests. Take the event where the harness delivers it into the session, and keep a scheduled self-wake as the backstop for the transitions delivery does not carry (in Claude Code, a pull-request activity subscription alongside send_later; in Codex, its own equivalents, and where a harness provides neither, end the turn instead of waiting) — see Phase 3; only when a machine event is stuck do you record state, end the turn, and wait for the human.
• The mandatory plan-approval gate — after the plan is written the run always stops for the human to verify it before any implementation (see Phase 1). Record the plan in the issue, mark the status block awaiting plan approval, and end the turn.
• A human decision with options — a Phase 1 Must-ask, an ambiguous review finding, or a conflict judgment call — asked inline through the question UI, with the answer returned in the same turn (see Asking the Human).

A harness that imposes a lighter posture does not exempt the change from the loop. When the runtime harness that launched the session frames the task as "just make the change, commit, and push," or restricts opening a pull request, treat that as a constraint on mechanics, not permission to skip the loop: still open the tracking issue and record the plan, still honor the plan-approval gate asynchronously — write the plan into the issue, end the turn, and wait for the resume — and still open the draft pull request. A harness clause like "do not create a pull request unless the user explicitly asks" is already satisfied: the host project's working agreement mandating a pull request for every change is the standing explicit ask, and the identical argument satisfies a harness policy conditioning a subagent spawn on the human's request — the same standing working agreement is that request too (see Delegated Implementation). Defer the pull request — and with it the independent review — only when creating one is technically impossible in the session, and a deferred independent review leaves the change not ready: report it as incomplete, never as done. Never let a generic "implement and push" instruction collapse the loop into self-approved completion.

Guidelines:

• MUST wait autonomously ONLY for machine events (CI, the review workflow); never keep a session alive polling for a human.
• MUST stop the turn and wait for a human resume at the plan-approval gate and whenever a machine event is stuck; resolve every other human decision inline through the question UI. Never schedule a self-wake to re-check for human input.
• MUST clear the Phase 1 clarify-before-building gate before writing the plan, and the plan-approval gate before implementing — never code against an unstated assumption or an unreviewed plan.
• MUST treat a conflicting runtime-harness posture — "implement, commit, and push," or a restriction on opening a pull request — as a constraint on mechanics, never as permission to skip the tracking issue, the plan-approval gate, or the independent review; a "no pull request unless asked" clause — and equally a clause conditioning a subagent spawn on the human's request — is satisfied by the host project's standing mandate, deferral requires technical impossibility, and a change whose independent review was deferred is reported as not ready, never as done.
• MUST treat the running session as the primary state store; write durable status to GitHub only as a recovery breadcrumb (see Run State and Reporting), not as the mechanism of record.
• MUST keep each externally observable step idempotent, so a resume re-reads state and continues rather than duplicating work.
• MUST keep judgment, human interaction, approval, GitHub delivery, and merge readiness with you whether or not implementation is delegated; a worker never becomes a second loop driver, and single-agent execution weakens no gate.

Asking the Human

Every human-gated decision with options goes through the harness's question tool (in Claude Code, AskUserQuestion; in Codex, request_user_input) and is answered inline in the same turn. Use it whenever the session exposes it, judged from the tools actually available rather than assumed from the host; only where no such tool exists does the decision go into the turn output and end the turn instead. The plan-approval gate is not one of these — it ends the turn (see Phase 1).

GitHub Operation Conventions

A GitHub-operation capability owns how an agent operates GitHub at all — the sanctioned tool channel, the agent-comment marker, and issue-versus-pull-request targeting. Consult it whenever a phase touches an issue, pull request, comment, or branch. What the loop adds on top of it is below.

• MUST read github-conventions.md before writing to an issue or pull request this loop owns, or before reading a body that may carry the status block or the canonical plan content.

Delegated Implementation

Delegation happens only where the harness already exposes an agent that qualifies for the role and where the harness permits the spawn — a determination the run makes on every run, landing on permitted, barred, or undetermined. Single-agent execution is a normal outcome, not a degraded one, whenever the determination settles that way: no qualifying candidate, a barred policy, or an undetermined policy the human declined or that could not be asked. No harness-specific agent definition, exact role name, or named model is required.

• MUST read subagent-delegation.md before resolving who runs any delegated phase or role, and before writing a project's own definition for an agent this loop spawns.

Intake — Identify the Unit of Work

Determine, from the conversation and the current repository state, which kind of target you are delivering, then enter the matching phase.

Guidelines:

• MUST, for an issue or a free-form target, treat opening the tracking issue as the loop's first observable action and a checkable entry condition: before planning or writing any code, confirm an issue anchors the run, and for a free-form request open one capturing it first. Being about to plan or implement with no issue open means the loop was already left — stop and open it, rather than reconciling it after the fact. (The open-pull-request resume target is already anchored by its pull request and any issue it links, so this condition does not apply there.)
• MUST, on a resume, reconstruct state from GitHub before acting — the plan in the issue, the open pull request, its CI status, the independent review's comments, unresolved threads, and the status block — and resume the one pending step, not restart from Plan.
• MUST NOT begin new work from a bare "continue" when there is nothing to resume and no handoff package; state that there is nothing to resume and ask what was meant.

Phase 1 — Plan

Turn the target into a buildable specification recorded in the issue. Two gates stop the run for the human before Code, in order: the clarify-before-building gate, then the plan-approval gate.

• the canonical plan content's boundary, the revision identity approval binds to, and the one normalization applied before comparing
• the amendment disposition for a plan that changes another issue's already-approved plan, the three conditions under which its own approval carries the amendment's, and the separate-approval fallback
• archiving the original description in a marked comment, and never composing a body from a sanitized read of one
• presenting and recording visual-change presentation options

Guidelines:

• MUST read plan-document.md before writing or revising the plan in the issue body, before comparing a plan revision's identity, and before writing a plan that amends another issue's already-approved plan.
• MUST establish the harness-permission determination from Delegated Implementation before the first Phase 1 investigation read a subagent could carry, on every run and regardless of whether a policy statement was noticed, landing on permitted, barred, or undetermined; where that determination needs a question, the single per-run question covers every role the run may spawn, the investigator included, so no later executor resolution re-asks it.
• MUST settle where a wide investigation read lands before making it, as the investigation step below directs, rather than letting the question surface once the payload is already in your own context.

Then step through the phase:

• Read the issue (or the tracking issue) and its full thread, classify the work — UI-bearing, implementation-only, exploratory, or mixed — and investigate the smallest useful code and documentation context before proposing a plan. Consult every project skill whose routing condition matches the surface, and research current external docs when behavior depends on a fast-moving framework or platform the project uses. Where a read is wide only for one conclusion — a broad code search, a long thread, a file tree merely being located in — route it per context-ownership.md rather than carrying it into your own context directly.

• Clarify before building — required gate. Investigation resolves how to build; it does not resolve what the product should do. Before finalizing the plan, list every open item the spec leaves and sort each one:

  • Settle-and-note — a fact the environment can answer: code, project conventions, documentation, or the output of a command. Resolve it by investigation and record the choice as a stated assumption in the plan.
  • Must-ask — a decision needing human judgment: a product outcome, a UX or interaction choice, a scope boundary or non-goal, empty/error/edge-case behavior, a data-model or persistence/migration decision, a trade-off between competing goods, or anything privacy-, platform-, security-, or compatibility-sensitive the issue does not pin down.

  If any Must-ask remains, you MUST NOT start implementing — put them to the human through the question UI (see Asking the Human), then use the answers to finalize the plan. Ask only genuine spec gaps, never what local investigation already answers. Where the project ships its own clarifying-interview practices, follow them for how the interview is conducted — question order, depth, and the restatement that closes the gate; in their absence, the gate clears once no Must-ask remains. A continuation that arrives while a Must-ask question is still open is a resume signal that re-presents that question, never an answer to it (see Never Manufacture the Human's Side).

• Rewrite the issue body into a comprehensive plan following the canonical plan-document structure and its section craft (above), in a single issue write. Refine the title to the concrete deliverable, and preserve the original description — collapsed inline, or in a marked archival comment where keeping it inline would leave no room for a later revision. Compose the body from text you authored, never from the current body read back through a sanitizing channel.

Phase 2 — Code + Verify

• Choose the working location before touching files. In a cloud environment the session already runs in an isolated, ephemeral checkout, so implement directly. In a local session sharing the human's working tree, implement on a separate git worktree so the run never blocks the human's own copy — unless the human explicitly asked to work in the current checkout. Either way, work on a branch under the harness's push-allowed prefix (an agent-namespaced branch such as claude/issue-<n> in Claude Code, or the prefix the running harness allows); never push to the default branch.
• Resolve who implements, before the first project-file edit. With the branch selected and approval in hand, first establish whether the harness permits a spawn at all, then resolve the executor per Delegated Implementation — a qualifying worker, or yourself in fallback. When delegating, package the task, grant the writer lease, wait, then reclaim the lease and check the receipt against repository state before any push.
• Implement strictly from the approved plan, keeping edits within the smallest surface that satisfies the acceptance criteria — yourself, or through the worker's package. Follow every project skill whose routing condition matches the changed files, and add or update the test coverage the plan named.
• Run the verification the changed surface requires — the project's format, lint, type-check, and test commands — and record the evidence (commands run, results) in the pull request body. When a required check cannot run, say so and note the residual risk rather than claiming it passed.
• Reviewer-mode self-check. Before opening the pull request, stop editing, reread the request, inspect git status and git diff, and review only the produced diff as if another author wrote it — fixing obvious Critical/Major issues. A delegated worker performs this on its own diff and reports it in the receipt; you then run the completion-evidence check against repository state rather than repeating the full review. Either way this is a self-check to avoid trivial hand-backs, NOT the authoritative review; that is the independent reviewer in Phase 3.
• Pre-flight review — advisory. Where implementation was delegated and the harness exposes a second worker that qualifies as a reader, one review-only worker judges the diff before the pull request opens, driving an implement→review loop until every finding it raises reaches a terminal state. It buys a reviewer that does not carry the implementer's reasoning state — as far as the reference's own write/clear pairing holds, never outright — and nothing else; it is not the independent review and never reported as one. With no compatible review worker the stage is skipped and the run continues from the self-check above.

Guidelines:

• MUST read pre-flight-review.md before resolving, running, or reading back a pre-flight review worker.
• MUST reuse the harness-permission determination Phase 1 already established for a run that passed through Phase 1, rather than establishing it again, and MUST establish it — regardless of whether a policy statement was noticed, landing on permitted, barred, or undetermined — for any run that did not pass through Phase 1, before whichever comes first of that run's first project-file edit and its first delegated fix. An open-pull-request target is such a run and reaches neither this phase nor Phase 1: Intake routes it to the Phase 4 tail, where a delegated fix is the first action a spawn licenses.

Phase 3 — Request Independent Review

Review is not done by you. It runs as a separate agent session on separate infrastructure — a different session under a bot identity distinct from the operator — so the code's author never certifies its own work. In Claude Code this is a CI workflow triggered by @claude review; in Codex, the equivalent triggered by @codex review. Either applies the project's posted-review policy and submits findings as inline comments anchored to the diff, tagged by severity. The independent review exists only as that separate-identity review submitted on the pull request; any assessment produced inside the authoring session is self-review, whatever it is called, and MUST NOT be reported as the independent review.

Guidelines:

• MUST read independent-review.md before choosing a waiting mechanism for CI and the independent review, and before addressing a review finding or a merge conflict.

Then step through the phase:

• Open the pull request in draft with Closes #<n>, structured from any repository pull-request template, summarizing the change, the verification evidence, and the acceptance criteria with their status. Seed the status block into the description as an HTML comment (see Run State and Reporting).
• Request the review by posting a top-level comment whose body is exactly the review trigger phrase — @claude review for a Claude Code reviewer, @codex review for a Codex one — plus the project's agent-comment marker line, and nothing else. Post the phrase the project's own reviewer answers to; posting both fires two reviews. Do not write that phrase anywhere else, or you will fire duplicate reviews.
• The review is a machine event that completes on its own — wait for it in the tail alongside CI. Do NOT review the diff yourself in its place.

Phase 4 — Address

Address the independent review's findings and CI to convergence, then gate the ready flip on the conditions independent-review.md states. Those conditions and the rest of the granular rules — resolving each thread against its fixing commit, re-requesting review, the round cap, and mergeability/conflict handling — live in that reference, routed from Phase 3.

Guidelines:

• MUST address and resolve each blocking finding and every unmet acceptance criterion, pushing fixes to the same branch and re-running the relevant verification after each batch.
• MAY delegate a mechanical CI failure or an unambiguous finding to an implementation worker, keeping ambiguous product and architecture findings — and every push, reply, and thread resolution — with yourself. A run that entered here without passing through Phase 1 or Phase 2 — an open-pull-request target — MUST establish the harness-permission determination before that first delegated fix, since it is the first action a spawn licenses on this path.
• MUST gate the draft→ready flip on every condition the independent-review reference states — never on your own assessment of your code, and never on a subset of them. On convergence, flip the pull request to ready, update the status block, and deliver the Ready-to-Merge Handoff. Merging remains the human's decision.
• MUST, when a human comments on a ready pull request, re-read the new threads on resume, address or escalate each, convert back to draft if needed, request a fresh independent review, and re-enter this loop as a new round.

Run State and Reporting

State lives in this running session; GitHub carries only a thin status block — invisible to a human reading the rendered page, fully visible to any agent, including this loop's own participants, that reads the raw body — so a resumed session can recover, and the turn that flips the pull request to ready doubles as the human's verification brief.

Termination Guard

An autonomous run has no natural stopping point: a review that keeps finding new problems, a check that never reports, and a human who never returns all look the same from inside the loop — like work still in progress. Each cap below names where continuing stops being progress, and what to leave behind when it does.

Guidelines:

• MUST cap the address↔review loop at 8 rounds; on non-convergence, record what still fails in the status block, state the summary in the turn output, and end the turn.
• MUST cap autonomous waiting at the awaited work's own declared timeout plus a margin wherever one is observable — a workflow's timeout-minutes, or whatever ceiling the platform states — and at 2 hours where none is, going dormant rather than waiting indefinitely; reset the budget when a check produces a result and a new push starts a fresh run.
• MUST cap delegated execution at one initial attempt plus 2 retries per approved plan revision and task phase, and recover in single-agent mode rather than spawning a fourth worker.
• MUST cap the pre-flight implement↔review loop at one initial implementation plus 3 autonomous rounds, then ask the human once per further round; this is a pre-pull-request cap and is distinct from the address↔review cap above.
• MUST NOT cap the Phase 1 clarify-before-building gate with a question budget — unlike the loops above, it is deliberately uncapped.
• MUST end the turn (never loop-block) whenever waiting on a human — the plan-approval gate, a stuck machine event, or a dormancy cap.
• MUST keep edits to the smallest surface that satisfies the acceptance criteria, never push to the default branch, and never merge the pull request.
