TypeScript build

• tsconfig.json targets CommonJS, strict mode. Source maps off (release artefacts, no stack traces needed since this is a CLI).
• src/**/*.ts compiles to dist/*.js. No bundler.
• Entry point for npm consumers is dist/cli.js (per main in package.json). bin/awesome-md-to-pdf.js and bin/md-to-pdf.js are CommonJS shims that simply require('../dist/cli.js').
