STEP 1 — Gather Information

Collect baseline sitting and standing blood pressure, heart rate, symptoms of orthostatic hypotension, current alpha-blocker dose and timing, any adverse effects, and confirm functional PPGL (catecholamine excess) via biochemical testing.

STEP 2 — Rule In / Rule Out

Is sitting systolic BP ≥130 mmHg or diastolic ≥80 mmHg? If yes, proceed to titration; if no, consider dose reduction or hold and reassess in 24 hours.

STEP 3 — Classify or Stratify

Classify response as: (a) BP above target without hypotension → increase dose; (b) BP at target → maintain dose; (c) symptomatic orthostatic hypotension → reduce dose or hold.

STEP 4 — Decide

Increase dose by 1–2 mg of doxazosin (or equivalent) every 2–3 days until sitting BP <130/80 mmHg is reached; if orthostatic hypotension develops, reduce dose by 50% or hold and re‑evaluate before further titration.

Concrete Clinical Example

A 48‑year‑old with biochemically confirmed PPGL starts doxazosin 2 mg at bedtime. After 3 days sitting BP is 150/94 mmHg, no orthostatic symptoms → increase to 4 mg nightly. After another 3 days BP is 138/86 mmHg → increase to 6 mg nightly. After 3 days BP is 126/78 mmHg and standing systolic is 92 mmHg → hold dose; continue same dose until surgery.
